Boilers can run for decades, which makes the repair-or-replace call different from any other appliance in the house.
Rule of thumb
A cracked heat exchanger or leaking boiler sections mean replacement; almost any other failure on a boiler with a sound block is worth repairing, even on an old unit.

Boilers age on their own timeline
Most heating equipment at twenty years is elderly, but a cast-iron boiler at thirty can be midlife. Hydronic systems have fewer moving parts in the heat path, and most of what fails are the components around the boiler rather than the boiler itself. That is why age alone should never condemn a boiler. The condition of the heat exchanger and the boiler sections is the real verdict, and a proper inspection can establish it.
The one failure that ends the conversation
Water where it should not be is the tell. A boiler leaking from its sections or heat exchanger, rather than from a valve, pump flange, or fitting, has reached the end, because the pressure vessel itself has failed. Everything else, from a dead circulator to a waterlogged expansion tank to a failed igniter, is a repair with a known cost. Insist on knowing exactly which kind of failure you have before agreeing to anything larger.
The efficiency question, without the sales gloss
Modern condensing boilers genuinely use meaningfully less gas than old atmospheric units, and across an Eastern Iowa heating season that difference is real money. But condensing boilers are also more complex, need annual service, and reach their advertised efficiency only when the system runs low return temperatures, which old high-temperature radiator systems do not always allow. An honest proposal addresses how the new boiler will actually behave with your radiators. Efficiency on a brochure is not efficiency in your basement.
Decide in October, not January
